Our team of health care experts uses your record to plan patient care, document communication and prepare for upcoming treatment or procedures.  Our Health Information Management (HIM) team protects the privacy and security of your health information and ensures your medical records are available for continuing care and other uses.

For Patients: How to request your records

Request records for personal use or continuing care

Patients who need to access their records for personal use or continuing care can easily access or request records via MyChart, our dedicated patient portal. MyChart allows you to download, save, print or request copies of self-downloadable health records, including:

  • Test results
  • Medication lists
  • After-visit summaries and clinical notes

How to request additional records

 If you do not wish to use our online Patient Portal to get access to your records, you can also request medical records by completing our Medical Record Release Form, and returning it to Health Information Management. The medical release form can be found by clicking this link

We are happy to send your medical records directly to other medical providers or facilities at no cost.  Records for legal, insurance and other purposes are subject to fees set according to KSA 65-4971(b) legal limitations.  Please expect to wait 7 to 14 days for completion of current medical records.  Older records may take longer.
