Sam McLemore DPT

Sam and her husband, Ryan, have three daughters and a dog. She loves playing sports with her girls, going on walks with her dog, hiking, and traveling. Her family enjoys going on road trips and having family movie nights. She is also active within the community and has served as a member of the Sacred Heart School Council, and is a current member of Sacred Heart Home & School and CCC PTA advisory board. She loves being able to coach her daughters in their youth volleyball, basketball, and softball teams. She enjoys working in her hometown and serving this community. Sam has been a physical therapist since 2010. She enjoys treating total joint replacements, orthopedic surgical repairs, balance impairments, and geriatric wellness for outpatient, inpatient, and skilled nursing populations.

Tina Harris, PT, DPT

Tina has 40+ years of experience in general orthopedics, hand therapy, neck and back pain, pelvic floor dysfunction, balance dysfunction, and generalized weakness. She is married to Cliff Harris, and has 2 fur babies (Riley, a Chocolate/German Shorthair cross, and Gizmo an American Fuzzy Lop).  She is an active community member, currently serving as Treasurer for the Thomas County Amusement Assn, is a Kiwanis and Sacred Heart Church member serving on the parish council and liturgy commission. Tina has served on the Colby-Thomas County Chamber Board of Directors, CCC PTA Advisory Board, AYSO Region 994 as regional commissioner, coach, and referee. When not working or volunteering, she enjoys spending time with her family and friends, cake decorating, camping, biking, hiking, paddle boarding, gardening, and playing pickleball.

Mackenzie Best, M.S.,CCC-SLP

Speech Language Pathologist

She is a Hoxie native and returned back to Hoxie after graduating. Mackenzie values faith, family, and friendship. She works hard to customize care plans for each individual using a person-centered approach. Her primary goal is to assist each individual in participating in daily activities. She offers a wide range of individualized treatments for all ages. Mackenzie is married to her husband, Heath, and they have three children. She likes to spend time with her family as well as cheer on the wildcats, play/coach sports, watch movies, and read.

Brittany Shields OTR/L, CLWT

Brittany is passionate about delivering patient-centered care to her work. She is excited to educate others about lymphedema and pelvic floor conditions to help people live more meaningful lives. Brittany is a Colby native and moved back to Colby after graduating from the University of Kansas in 2016. She met her husband, Justin, shortly after moving to Colby. Brittany and Justin got married in November of 2019 and have since welcomed 3 children into their family. Brittany enjoys spending time with her family, doing craft projects with her children, vacationing at the lake and playing volleyball.

Derick Bacon PTA, FMS 1- 2

Derick grew up in in Overton NE, and moved to Colby for the PTA program at the Colby Community College where he met his wife Jennifer. After graduation, they moved to Grand Island, NE and worked there for 7 years before moving back to Colby in 2017. They have 3 girls, Jordan, Caylee, and Rylynn and a son Dawson. Derick is an avid Nebraska football fan. In his spare time Derick enjoys giving back to the community by coaching Football and Track for Colby High School, along with spending time with family at the lake, boating, waterskiing, and playing yard games.

Jennifer grew up in Dodge City, and moved to Colby for the PTA program at Colby Community College where she met her husband Derick. After graduation, they moved to Grand Island, NE and worked there for 7 years before moving back to Colby in 2017. They have 3 girls, Jordan, Caylee, and Rylynn and a son Dawson. Jennifer enjoys watching KU basketball, playing volleyball and swimming, as well as being outdoors with her family hiking, snow skiing, and being on the water.

Jeremiah grew up in Hoxie, KS working on his family's farm. He attended Colby Community College for the PTA program in 2011. After graduation, he worked as a PTA in Colorado Springs, CO - for 8 Months, Hays, KS - 2 Years, and in Overland Park, KS - for 4 years before deciding to come back to his roots and settle down in Northwest Kansas. Jeremiah is an avid follower of Hoxie High School Sports, as well as the KSU Wildcats, Kansas City Chiefs, and Kansas City Royals. He enjoys spending time with family at the lake, enjoys going to concerts, and sporting events.

Paul is originally from Colby, KS and is married to his wife Torie. They have 4 children: Joseph, Sebastian, Philomena and Kolbe. His hobbies include spending time with his family, watching his kid’s activities. Paul is also an assistant football and track coach at Colby High School.

I was born and raised in Colby. I attended CCC and graduated with an associate degree in applied science. I have been a CNA, CMA, and CRA. I have also worked as an office manager for a local trucking company before moving to Hays. Upon moving back to Colby, I started working for Tina Harris Physical Therapy as office manager. I am married to the love of my life, Matt Hayes. We have 3 children. Morgan Smull, Dakota and Zach Redwing and Austin and Marie Hayes. We are the proud grandparents of Milo Hayes. We also have our fur babies Rip and Nala. I enjoy spending time with my family and friends. I enjoy watching football and NASCAR with my husband. GO BRONCOS!! We enjoy volunteering at our church and helping at the racetrack. I enjoy cooking and baking. Every once in a while, you will find baked goodies near my desk for others to enjoy.

Sara has been with Citizens Health since 2008. She started in housekeeping for 2 1/2 years before transferring to the Rehab team where she found her love of helping patients. She was a tech in the department for 7 years before she became the receptionist.  Sara is married to Jory and they have 2 children, Evan and Kyra. She loves to spend time with family and friends, watching/participating in demo derbies and being outdoors during the nice summer days soaking up that Vitamin D.
