What should I bring for my hospital stay?

  • Insurance card and photo ID
  • Contacts (including case and solution) or glasses
  • Hearing aids
  • Dentures and denture adhesive
  • Comfortable clothing and shoes
  • Items for entertainment during pre-op.]
  • Current medication list (prescription and nonprescription), including inhalers, eye drops, vitamins, supplement and herbal medications. (Bring the actual medications with you, if it’s impossible to make a list.)
  • Medical history information, including allergies and surgical history
  • Immunization history, especially including tetanus, flu and pneumonia immunizations
  • If you use a CPAP machine, please bring along with a diagnosis of Sleep Apnea
  • Copy of advanced directives or living will
  • Emergency contact name and phone number
  • A list of questions to ask your health care providers about your procedure or care

What should I leave at home when preparing for a hospital stay?

What are visiting hours and rules?

  • We encourage visitation between 10:00 a.m. and 8:00 p.m. daily, however, visitors may also visit outside of those hours if needed.
  • Please delay visitation if you are ill.
  • Children under 12 years of age must be accompanied by an adult.
  • Wash your hands upon entering and exiting patient rooms.
  • No smoking allowed on the Citizens Health campus.
  • Cell phone use is allowed.
  • Free Wi-Fi is available by connecting to Citizens-Public. No username or password is required.

Family Center for Health Care

What are the office hours for the Family Center for Health Care?

Regular office hours for the clinic are:

  • Monday – Thursday: 7 a.m. to 6 p.m.
  • Friday: 7 a.m. to 4 p.m.

Walk-in Hours:

  • Monday – Friday: 7 a.m. to 9 a.m.

What should I bring to my appointment at the clinic?

What are some tips for preparing for my appointment?

  • Plan to arrive 15 minutes early for your scheduled appointment to allow us to update your information and prepare for your visit.
  • Parents or legal guardians should accompany minor children to their appointments.
  • Transfer medical records to us prior to your first visit, if you’re transferring from another medical provider.
  • Bring a list of your current medications.
  • Inform us of any changes to your medical history, including allergies or surgeries.
  • Keep us updated with name, address, phone or insurance information changes.

What should I do if I have questions about medical care after the clinic is closed?

Established patients or their caregivers can call (785) 443-5171 and speak with a medical provider to assist with issues that occur beyond office hours. If you are having an emergency, call 911 or go to the nearest emergency room immediately.

What is the Special Care Unit at Prairie Senior?

Our Special Care Unit is designed for patients with Alzheimer’s disease or other forms of dementia.  The unit is staffed by professionals who specialize in care for residents with these medical diagnoses.

What are the visiting hours at Prairie Senior?

We encourage our residents to have visitors anytime. We are available to assist in making any visit more enjoyable.

What are the accommodations like at Prairie Senior?

Prairie Senior is a 60-bed facility composed of 34 Long-Term Care, 12 Special Care Units, and 14 Residential Care beds. We offer the level of care that is required by our residents today, but we are able to provide more comprehensive care, if needed, in the future.

We seek to provide a home-like atmosphere that allows our residents to live an enjoyable and independent lifestyle. We want our residents to enjoy our open floor plan and living environment.

 

Can I volunteer at Prairie Senior?

We are always looking for people who would like to volunteer their time and share their talents, kindness, and concern with others. Anyone who can give the gift of time can volunteer.
