Adolescent Health

Health Maintenance

Lifetime habits may form during adolescence and prevention of health problems at a young age can directly influence the health experienced in adulthood. This is why it’s important to seek regular preventative visits that reinforce positive health habits and discourage negative health habits.

Many of the causes of morbidity and mortality in adolescence are preventable health conditions and are related to behavioral, environmental, and social causes.

Regular visits can help prevent or detect obesity, hypertension, smoking, substance abuse, unsafe sexual practices and pregnancy, improper nutrition, depression, and other health problems and maintain proper immunization status.

 

Cervical Cancer Prevention

Two vaccines (Cervarix and Gardasil) are available for use in preventing Human Papillomavirus (HPV) and HPV related diseases. Both vaccines are available for females, only Gardasil is available for males. Gardasil has been shown to protect against genital warts and against cancers of the anus, vagina, and vulva. The vaccines are indicated for girls as young as age 9 but are typically recommended for 11 to 12-year-old girls and girls age 13 to 26 who have not previously been immunized. Gardasil is recommended for all teen boys and men through age 21. These vaccines offer the greatest benefit if all three (3) doses are administered prior to any type of sexual activity.

 

Birth Control

Birth Control can be an effective strategy to prevent unintended pregnancy, reduce menstrual cramp pain, and help regulate menstrual cycles. Several methods exist and differ by their prevention method. Methods include intrauterine contraception, hormonal, or barrier.

  • Intrauterine contraception is accomplished by the insertion of an intrauterine device or system.
  • Hormonal contraception can include implant, injection, oral contraceptives, patch, or vaginal contraceptive ring.
  • Barrier methods include male or female condom, diaphragms, or spermicides.

 

Sports Physicals

A pre-participation physical exam is required for participation in school sports programs and ensures that it is safe to participate. You should be given a form from your school for use by the medical provider during this exam that you will then return to the school.

The exam consists of two parts – a medical history and physical exam.

The medical history includes questions related to medication use, health problems, past injuries or hospitalizations, surgeries, allergies, and general and family health conditions.

The physical exam will include height, weight, vital signs, vision, physical assessment, and check your overall appearance.

Aesthetic & Cosmetic Medicine

Family Center for Health Care offers aesthetic and cosmetic medical services you would typically expect from only plastic surgeons. Our providers meet with patients one-on-one to determine their cosmetic goals and answer questions. They then provide a recommended treatment that will be the most effective and have the best outcome for each individual. Our providers offer several non-surgical methods to help you regain your natural and youthful looking beauty. Since these services are non-surgical they offer you the ability to judge the results for yourself and discuss potential treatment modifications.

 

Botox®

Are you concerned about deepening frown lines between your eyes?  Have you noticed wrinkles appearing on your forehead or between or at the corners of your eyes? If so Botox® may be right for you.

Botox has been approved by the FDA as a safe and effective treatment for wrinkles and facial lines. Botox can also be used in the treatment of headaches, limb spasticity, cervical dystonia, strabismus, blepharospasm, and urinary incontinence.

Restylane®

Have you begun to see wrinkles on your face or around your lips?  Have your lips begun to lose their plumpness and volume?  Restylane® may be the solution. It is a dermal filler used to restore the volume and fullness for moderate to severe facial wrinkles and folds. The benefit from this treatment is immediate and is achieved in a safe and gentle manner. Each treatment will last six to twelve months depending on your individual body chemistry.  Restylane can be used on facial wrinkles and folds around the eyes, nose, and mouth or for lip augmentation and acne blemish correction.

Radiesse®

Radiesse® offers patients another treatment choice when looking to soften and smooth their facial wrinkles. Radiesse Volumizing Filler is an injectable implant composed of synthetically produced smooth calcium hydroxylapatite microspheres that offers the benefit of a dermal filler combined with the ability to promote your body’s own ability to produce collagen. This ability to promote your own collagen production can lead to extended benefits and increase your satisfaction. Each treatment may last a year or more. The effect is however not permanent.

Chemical Peels

Chemical peels remove the outer layers of skin to expose new layers beneath. The procedure involves the application of a chemical solution to the skin and may contain a variety of contents and concentrations to achieve the desired results.

 

PCA Products

PCA SKIN® Clinical Care Products and PCA® MEN products are scientifically formulated using technically advanced, all-natural ingredients to nourish, hydrate, and rejuvenate your skin. Whether due to acne, rosacea, sun damage, or aging skin; daily use will dramatically improve skin health and help provide the glowing, radiant complexion you desire. There are a variety of cleansers, toners, moisturizers, treatment, and control products available. PCA home care products are free of color additives, synthetic fragrances, harsh alcohols, comedogenic oils, and other known sensitizers and are designed to deliver results without irritation.

 

Naval Piercing

Men and women can seek body piercing as a method of self-expression and cosmetic improvement. The piercing procedure does require sterile technique and our medical providers have education and experience performing procedures while maintaining sterility. Failure to maintain sterile technique can cause disease transmission that includes: Hepatitis B, Hepatitis C, tuberculosis, syphilis, and HIV. Common complications include infection, scarring, metallic allergies, and the piercing catching on clothing. FCHC offers naval piercing to its patients. You are asked to bring a 14 or 18 gauge barbell (preferably curved) made of surgical implant stainless steel, Titanium, 14K or 18K gold, platinum, or niobium. Most people receiving body piercings have no problems.

Allergy

Allergies are a common occurrence in most people however they are often unique to each person in the manner they exhibit themselves. Allergies affect the immune system by causing irritation or mistaking allergens for harmful substances within the body. They are treatable and you should consult your physician to find a medical treatment that is right for you.

Anesthesia

Anesthesia is a medication given in order to perform surgeries and medical procedures that would otherwise cause discomfort or pain. Anesthesia can be given through breathing masks or tubes and intravenously through IV sedation.

Anesthesia has to be administered by a specialized care team to include an anesthesiologist or nurse anesthetist, as well as trained physicians and nurses to monitor vital signs.

Asthma

Asthma is a chronic disease of the lungs where it is difficult to move air in and out. Asthma can affect you at any age and is known to come later in life. Although there is not a cure for asthma there are treatments for management to meet lifestyle expectations.

Symptoms include:

  • Shortness of breath
  • Wheezing or whistling sound when you breathe

Chronic Disease Management

Dermatology

Abnormal skin growths can indicate the presence of a disease of the skin or can simply impair your cosmetic appearance. A proper evaluation can determine whether skin revision (removal) and/or skin biopsy should be performed. The skin biopsy will allow us to microscopically examine and diagnose any cutaneous (skin) conditions.

Dietitian

Dietitians are trained in providing advice and education in regards to your nutrition and creating a healthy balanced diet. Their goal is to promote healthy eating to prevent or treat illnesses caused by excessive or undernourished eating habits and/or food allergens. A dietitian will conduct an assessment based off of your current health and health history and create an individualized plan to meet your nutritional needs that will lead to a happier and healthier life.

Geriatrics
Medicare

Our initial preventative physical exam is for new enrollees in Medicare and is a one-time comprehensive preventive visit benefit per Medicare Part B. You are eligible to receive this exam during the first twelve months you are enrolled in Medicare Part B.

The exam offers many screening, wellness, prevention, and immunization benefits to you which are designed to promote and maintain your health. The screening program allows us to screen for heart disease, diabetes, glaucoma, osteoporosis, certain cancers, nutrition deficits, and other health problems.

Please talk to us so we can help you understand the benefits you are eligible to receive. If you have secondary insurance, a coinsurance or copayment may still apply.

 

Annual Wellness Exam

You are eligible to receive an annual wellness exam once every twelve months as part of your Medicare Part B benefits. This yearly visit is used to develop or update a personalized prevention plan based on your current health and risk factors. It must have been twelve months since you had your “Welcome to Medicare” visit. You don’t need to have a “Welcome to Medicare” visit before your yearly “wellness” visit.  All people with Medicare are eligible to receive this benefit.

You pay nothing to receive this benefit if your doctor or health care provider participates in the Medicare program. If you receive additional tests or services during the same visit and these are not covered under preventive benefits, you may have to pay coinsurance, and the Part B deductible may apply.

 

Nursing Home Program

Our nursing home program allows you to receive health care services from us at the living complex of your choice. We provide you with convenient and regular health care visits from our medical providers.  These visits are designed to monitor and maintain your overall health.

We are in regular contact with the nursing staff to help you through any acute problems as they may occur and are able to provide care to patients who experience health care problems at times outside of normal office hours. This helps to eliminate requiring travel from your living complex to visit your health care provider.

This service is offered to the residents of Prairie Senior, Deseret Health & Rehab at Colby, and Fairview Estates Retirement Community.

Gynecology

Our gynecologists are specialized in addressing women’s health issues related to childbirth, family planning, and cancer prevention. Scheduling regular pap smears with your primary care provider or gynecologist can help in the early detection and/or prevention of certain diseases like breast and cervical cancer or certain STDs.

Hearing and Vision Screenings

Hearing Screenings

Hearing problems can be caused by either sound transmission or nerve problems. Our hearing screening exams can help determine if and what type of hearing problem exists and then determine the best method of managing the hearing impairment.

 

Vision Screenings

A vision screening exam can be used to detect the presence or potential of a vision problem. The exam determines if there are any pre-existing health problems or familial health problems that are more likely to predispose you to vision problems. It includes an evaluation of your eyes, visual acuity testing, and visual field testing. The results are discussed with you and a plan is developed with you that includes either observation, further evaluation, or referral to another medical professional.

Vaccines for Children Program

The Vaccines for Children (VFC) Program offers vaccines at no cost for eligible children through VFC-enrolled doctors. Children through 18 years of age who meet at least one of the following criteria are eligible to receive VFC vaccine: Medicaid eligible, uninsured, American Indian or Alaska Native, or underinsured.

Adult Vaccination

The need for vaccinations does not end once you enter adulthood.  The immunization process continues throughout life in order to maintain good health and prevent disease. Talk to your health care provider about what immunizations you should receive in order to maintain a proper immunization status.

 

Flu

Anyone can get influenza (flu) virus. Most people are ill with influenza for only a few days but in some people, it can cause serious illness, hospitalization, and even death. There is an average of 12,000 – 56,000 deaths each year in the United States, most occurring in the young and the elderly.

There are two methods used to immunize people currently, an injectable form and a nasally administered form.  We follow current CDC immunization guidelines in determining which form to recommend and administer to you.

In addition to receiving your flu shot using these strategies can help you reduce your risk of getting the flu:

  • wash your hands or use alcohol-based hand cleansers often
  • avoid contact with people who are ill
  • avoid touching your eyes, nose, or mouth, cover your mouth and nose when coughing
  • stay home if you develop flu-like symptoms.

 

Pneumococcal (Pneumonia)

Pneumococcal disease is an infection anyone can get. In fact, pneumococcal infections cause more deaths each year than all other vaccine-preventable diseases combined. Most often infection occurs in the lungs, blood, or brain. These infections which were once easily treatable with antibiotics have become complicated by antibiotic resistance. This makes vaccination even more important.

The Pneumovax® vaccine provides you with valuable protection.  Commonly called the “pneumonia shot,” it does not keep someone from getting pneumonia or a chest cold. However, it does prevent the pneumococcal disease from causing a secondary infection or a more serious life-threatening illness.

 

Tetanus & Pertussis

Tetanus (lockjaw) is a serious disease that causes a painful tightening of the muscles, usually all over the body. It can lead to “locking” of the jaw so the victim cannot open his mouth or swallow. Tetanus leads to death in about 1 in 10 cases. Several vaccines are used to prevent tetanus among children, adolescents, and adults including DTaP, Tdap, DT, and Td.

Whooping cough, known medically as pertussis, is a highly contagious respiratory tract infection. Although it initially resembles an ordinary cold, whooping cough may eventually turn more serious, particularly in infants. It is most contagious before the coughing starts.

The best way to prevent this infection is through vaccination. For children the vaccine is called DTaP. The booster vaccine for adolescents and adults is called Tdap. Both protect against whooping cough, tetanus, and diphtheria.

 

Travel

When traveling to remote locations across the globe there are health concerns and diseases not present in or have been eradicated from the continental United States. Safe travel may require you receive additional immunizations to protect you.

The time to administer these immunizations can require several months. You should contact us as soon as you consider traveling to these locations to schedule a visit and begin the vaccination process.

Men's Health

Men’s Preventative Health

It is important to establish regular visits with your medical provider to help reduce your risk for health problems and in some cases prevent the development of health-related issues. These visits can help prevent the two leading causes of death for men of all ages heart disease and cancer.

When early detection occurs your chances for a cure or effective treatment is increased. This can help you lead a longer and healthier life by developing a schedule of exams, tests, and screenings. They may include running the following test and screens:

  • cholesterol
  • blood pressure
  • blood sugar
  • prostate cancer
  • colorectal cancer
  • skin cancer

Regular visits will also allow for updating your immunizations. We recommend you make an appointment if you become sick, have pain, a change in your health, or problems with your medications.

As part of a healthy lifestyle, we recommend a balanced diet that leads to a healthy weight by choosing a diet composed of plenty of fruits, vegetables, and whole grains. You should limit foods and drinks high in calories, sugar, salt, fat, and alcohol. Physical activity that increases your heart rate and breathing for at least 2.5 hours a week can reduce your weight and blood pressure. It can also reduce your risk of diabetes, heart disease, stroke, arthritis, some cancers, osteoporosis, depression, and anxiety.

Additional healthy habits to practice:

  • Avoid or quit smoking
  • Limit use of alcohol
  • Get a recommend 7-9 hours of sleep per night
  • Develop an effective stress management plan
  • Maintain a positive outlook on life

 

Prostate Health

A majority of men will experience prostate related health problems during the course of their life. Three problems are regularly encountered, include:

  • benign prostatic hyperplasia (BPH or enlarged prostate)
  • prostatitis
  • prostate cancer

These conditions can cause the following symptoms:

  • frequent urination
  • difficulty urinating
  • weak stream
  • incomplete emptying of the bladder

Prostate Cancer is the most common cause of cancer in men. Regular exams and early detection of prostate cancer can lead to a more successful treatment outlook.

 

Colonoscopy

A Colonoscopy is a procedure used to visualize the colon (large intestine) and detect inflamed tissues, ulcers, and abnormal growths. Providers often utilize this procedure for exploratory and diagnosing purposes, however, they may also remove abnormal growths and perform tissue biopsies.

This procedure may be recommended if you experience any changes in bowel habits, abdominal pain, bleeding from the anus, or weight loss. It is an outpatient procedure and can be performed with the aid of sedative and pain medications.

 

Vasectomy

A Vasectomy is a safe and effective form of male contraception. The procedure works by preventing sperm from exiting the testes. When considering a vasectomy you should consider its permanence as a form of birth control. The procedure is a simple outpatient procedure and patients often require only a short recovery period.

Otolaryngology (ENT)

Our specialists treat a range of disorders in the ears, nose, throat, head, and neck. Whether prescribing medications or developing a treatment plan, ENT providers can also perform extremely delicate and highly specialized surgeries.

 

Earwax Removal

Cerumen (earwax) is beneficial because it protects the eardrum from damage by dust particles or other small objects. When earwax does not dry up and fall out as normal then it can decrease hearing, cause pain or a full feeling, dizziness, or ringing in the ear among other effects.

Over the counter methods to remove earwax can be effective but when they are not then we can employ several methods to remove:

  • by curette (plastic spoon)
  • warm water
  • saline irrigation
  • prescription ear drops

We recommend against the use of cotton-tipped applicators or Q-tips for cleaning of the ear due to it likely pushing wax deeper into the ear or causing damage. If you wear hearing aids or earplugs you are at higher risk for cerumen impaction (excessive earwax build up).

IV Therapy

Intravenous therapy requires the insertion of an IV cannula into a vein, typically in either upper extremity.  Primary sites include the hand and forearm.  This allows the infusion of IV fluids, medications, radiological contrast, blood products, chemotherapy, and nutritional preparations.

Pain management focuses on diagnosing and treating chronic pain through medication and therapies similar to Epidural Steroid Injections. The goal of pain management is to reduce chronic daily pain and find ways for you to manage your pain and raise your quality of life.

Pediatrics

Well Child Check/Kan Be Healthy

Treating pediatric patients is something enjoyed by all the medical providers at FCHC. Your child’s visits are scheduled beginning one week after birth and continue on a regular basis through to adolescence years to ensure your child enjoys a healthy life, receives appropriate immunizations as recommended by the CDC, and receives appropriate medical treatment for any health problems encountered.

A typical well-child visit schedule may consist of visits at the following ages: birth, one year, two years, four years, six years, nine years, twelve years, fifteen years, eighteen years, twenty-four years, thirty months, and then each year from age three through twenty.  Our medical providers are able to provide your child with physicals required by schools, daycares, and camps.

Our medical providers participate in Medicaid’s Kan Be Healthy Program.

 

Asthma

Asthma is the most common chronic disease encountered in childhood. The disease is characterized by airway inflammation, excessive mucus production, and hyperresponsive airways.

Symptoms of asthma include chest tightness, non-productive cough, and wheezing.

Treatment can include short or long-acting bronchodilators, inhaled or systemic corticosteroids, and leukotriene modifiers.

The goals of therapy are to reduce the frequency of asthma exacerbations, the severity of exacerbations, and allow a healthy or near healthy lifestyle.

 

Ear Infections

After Upper Respiratory Infections (URI), Otitis Media (OM, ear infection) is the most common childhood illness.

Signs and symptoms typically include

  • otalgia (ear pain)
  • otorrhea (ear drainage)
  • fever
  • irritability
  • recent onset anorexia
  • vomiting
  • diarrhea

The diagnosis is confirmed by otoscopic examination allowing visualization of the tympanic membrane (TM, eardrum). The presence of tympanic membrane opacity, bulging, erythema (redness), or effusion can lead to a definitive diagnosis. Treatment may include close observation, antibiotics, and pain control (Tylenol or ibuprofen).

 

School Physicals

A pre-participation physical exam is required for participation in school sports programs and ensures that it is safe to participate. You should be given a form from your school for use by the medical provider during this exam that you will then return to the school.

The exam consists of two parts – a medical history and physical exam.

The medical history includes questions related to medication use, health problems, past injuries or hospitalizations, surgeries, allergies, and general and family health conditions.

The physical exam will include height, weight, vital signs, vision, physical assessment, and check your overall appearance.

PFTs can be used to

Other uses include preoperative risk assessment and occupational surveillance. The test measures the performance of the lungs, chest wall, and respiratory muscles by measuring the amount of forcefully exhaled air.

Weight Loss

Obesity is a major health crisis in the United States and in the rest of the industrialized world. It has been shown to be associated with other health problems that can significantly increase the risk of illness and death like type 2 diabetes, hypertension, high cholesterol, coronary artery disease, stroke, thrombophlebitis, gastroesophageal reflux disease, gallbladder disease, sleep apnea, osteoarthritis, some cancers, and more.

Our Weight Loss program utilizes several tools to assist you in reducing your weight and body fat concentration through nutritional counseling, exercise, medical condition management, and medical supervision.

Nutritional counseling allows you to determine what your caloric intake should be based on your metabolism and exercise level. This counseling will provide recommendations as you progress through your weight loss program. Our dietician will advise you on how to structure your meals effectively while eating nutritious and safe meals.

The Rehabilitation department provides a fitness area that allows you to exercise while receiving guidance from the rehabilitation staff. This allows you to exercise in an effective and safe manner.

Medical management and supervision allows your progress to be measured and quantified, thereby providing feedback to you on your progress and ensuring your health is improved through your weight loss journey while health problems are monitored and treated during the program.

A normal body fat concentration is in the range of 15-20% for men and 25-30% for women. Obesity is defined as a body fat concentration greater than 25%for men and greater than 35% for women.

The Body Mass Index (BMI) can also be used to determine whether someone is overweight or obese. The index is highly correlated with body fat percentage based on height and weight measurements. A BMI in the range of 25-29.9 signifies someone overweight, 30-39.9 obese, and greater than 40 signifies morbid obesity. Muscular people may have a BMI that inaccurately measures their body fat concentration.

Women's Health

Annual Exams

It is important to establish a schedule of regular visits with your medical provider. These visits can help reduce your risk of health problems and often times prevent the development of heart disease and cancer which are the leading causes of death for women of all ages.

When early detection occurs the chance for a cure or effective treatment is increased, leading to a longer and healthier life. We also recommend you see us if you become sick, have persistent pain, a change in your health, or problems with your medications.

Scheduled exams may include screening for and checking the following:

  • cholesterol
  • blood pressure
  • blood sugar
  • cervical or uterine cancer,
  • colorectal cancer
  • skin cancer
  • updating your immunizations

Breast and cervical cancer screenings should be performed during your annual wellness visit.

Breast cancer can be screened for using a simple breast exam or a mammogram.

Cervical cancer can be screened for using the PAP test (smear) which is used to screen for precancerous cell changes. Cellular changes on the cervix may become cervical cancer if they are not treated appropriately. The Pap test is recommended for all women.

The human papillomavirus (HPV) test looks for the virus that can cause these cell changes.

As part of a healthy lifestyle, we recommend a balanced diet that leads to a healthy weight by choosing a diet composed of plenty of fruits, vegetables, and whole grains. You should limit foods and drinks high in calories, sugar, salt, fat, and alcohol. Physical activity that increases your heart rate and breathing for at least 2.5 hours a week can reduce your weight and blood pressure. It can also reduce your risk of diabetes, heart disease, stroke, arthritis, some cancers, osteoporosis, depression, and anxiety.

Additional healthy habits to practice:

  • Avoid or quit smoking
  • Limit use of alcohol
  • Get a recommend 7-9 hours of sleep per night
  • Develop an effective stress management plan
  • Maintain a positive outlook on life

 

Birth Control

Birth Control can be an effective strategy to prevent unplanned pregnancy. Several methods exist and differ by their prevention method including intrauterine contraception, hormonal, or barrier.

  • Intrauterine contraception is accomplished by the insertion of an intrauterine device or system.
  • Hormonal contraception can include implant, injection, oral or patch contraceptives, or a vaginal contraceptive ring.
  • Barrier methods include male or female condom, diaphragms, or spermicides.

Bone Densitometry (DEXA)

Osteoporosis can be caused by various things. It consists of a loss of bone mass and a deterioration of bone tissue leading to fragile bones. Several treatments are available for this highly preventable disease however it is not usually detected until after a bone fracture has occurred. Early detection can take place by conducting a Bone Densitometry scan which measures bone mineral density and bone loss, thus determining fracture risk. Women are at greater risk for osteoporosis and should have frequent discussions with their medical providers.

Breast Cancer

The risk of developing breast cancer is largely determined by your medical and familial history. Breast cancer patients have benefited from increased detection and decreased mortality over the last several decades. An exam can help you determine your breast cancer risk and the appropriate frequency of screenings. Once detected, a process is followed which begins with determining the presenting symptoms, discussing clinical history, and then imaging and biopsy are considered for further discovery. Many early breast cancers cause no symptoms and are either not detected or larger ones are discovered as a lump during self-breast exams.

Mammograms

Along with a clinical breast exam performed by your medical provider, mammography is used to screen and detect breast cancer. This procedure takes an X-ray photo of your breasts to allow providers to look for any concerning signs of lump presence within your breast tissue.

 

Hormone Replacement Therapy

In the years preceding and following menopause the levels of estrogen and progesterone circulating in a woman’s body can decrease due to decreased production by the ovaries. The effects of menopause can have physical and mental effects. Hormone therapy can be used to manage the symptoms of menopause. It can be prescribed in forms that include gels, creams, oral medications, patches, and implants.

There are also several preparations and dosing regimens that can be utilized, however, there are contraindications to hormone therapy so therapy should be discussed with your medical provider.

Sexually Transmitted Diseases

To ensure your health we recommend, at a minimum, annual testing for sexually transmitted diseases. Certain STDs can have no symptoms and can cause significant health problems and affect your fertility. This screening can be performed as part of your annual exam.

Wound Care

Wound care is needed when lacerations occur on the skin where friction, shearing, compression, or traction tear or open your skin. Proper evaluation and repair can prevent infection, loss of function, and poor cosmetic outcomes. A proper evaluation can also detect and treat any underlying injuries that are incidental to the laceration. Your immunization status should also be determined and updated if necessary.
